Just installed after seeing an article on the local paper. Chose the free version for now but may upgrade if I like it. UI feels clean and easy to navigate. Only suggestion for improvement thus far is when choosing mobility to provide examples of what you can do, vs what is hard to do. I like the layout of the menu, and the idea of my doctor eventually being able to link in.

Freezes. Won't let you pick your choice

Have yet to sign up.... after 15 minutes of trying to scroll back to 1964 to enter my birthday, I accidentally touched outside the box, and now I have to start over.... Just allow people to type on their birthday, I may try again later, but this takes too long.
